1) Basic Home Bundle

• * Developed over 4 weeks to create an individualised home plan that is managed by yourself, family member or friend.

2) Reassurance Bundle

• * Developed over 4 weeks to create an individualised home plan that is managed by yourself, family member or friend.

• * This Bundle may include assistance with specialised support services i.e. linking into dementia services, personal care services, allied health etc.

• * This Bundle offers an ongoing monthly check in (one hour) with a guide from LiveWell at Home to ensure your Home Plan is supporting current needs.

• * Option for a yearly review meeting to ensure your Home Plan is reflecting your support needs.

3) Premium Bundle

• * Developed over 4 weeks to create an individualised home plan that is managed by yourself, family member or friend.

• * This Bundle may include assistance with specialised support services i.e. linking into dementia services, personal care services, allied health etc.

• * This Bundle offers an ongoing two weekly check in's (one hour each fortnight) with a guide from LiveWell at Home to ensure your Home Plan is supporting current needs.

• * Option for a six-monthly review meeting to ensure your Home Plan is reflecting your support needs.

• 4) Hourly Advisory Service

• Based on an hourly rate.
